• Evidence of choice of Institution or Evidence of change of Institution.
• UTME Result
• O’Level Results
• Two (2) recent passport sized photographs, preferably colored white background.
• Evidence of birth certificate from National Population Commission, or Age Declaration.

This is to inform all the prospective candidates for admission into College of Nursing, Amichi, for National Diploma in Nursing Programme that the documentation for Post UTME screening commences as follows:
